(ENSG) owns and operates independent subsidiaries that provide a broad spectrum of post-acute healthcare services across the United States, including skilled nursing; assisted and independent senior living; short- and long-term rehabilitation therapy comprising physical, occupational and speech-language pathology services; specialty medical care such as on-site dialysis, ventilator care, cardiac and pulmonary management; ancillary services encompassing mobile x-ray, ultrasound, electrocardiograms, respiratory therapy, long-term care pharmacy and non-emergency patient transportation; and real estate leasing of healthcare facilities through its Standard Bearer segment. The company serves patients with chronic conditions, prolonged illnesses and the elderly, primarily funded by Medicare and Medicaid, and targets communities in underserved areas with resident-centered care delivered via local management teams. Founded in 1999 and headquartered in San Juan Capistrano, California, Ensign operates over 370 healthcare facilities, including 300-plus care affiliates across 17 states such as California, Colorado, Arizona, Kansas, Tennessee, Oregon, Alaska, Washington and Alabama. Recent expansions include the December 2025 acquisition of four skilled nursing facilities in Colorado, Kansas and Arizona, increasing its portfolio to 373 operations; entry into Oregon and Alaska via eight facilities from Providence Home and Community Care; additions in Tennessee and Alabama; and multiple transactions involving joint ventures with CareTrust REIT, Inc. and purchases by Standard Bearer Healthcare REIT, Inc., alongside projections for record $5 billion in 2025 revenues.
